PCOR-QLD

For Participants

If you live in Queensland and have been diagnosed with prostate cancer, by law your diagnosis will be sent to the Queensland Cancer Registry.

If your hospital and clinician are contributing data to the Prostate Cancer Outcomes Registry – Queensland (PCOR-QLD), you may receive a Participant Brochure from your doctor and, some time after your diagnosis, you will receive a Participant Information Sheet and a letter of invitation to participate in PCOR-QLD. The Participant Information Sheet explains the study in detail and includes instructions on how to opt-out of the Registry, if you do not want to participate.

If you do not opt-out, you have provided consent for your relevant personal and clinical information to be collected and made available to PCOR-QLD. Your details will be collected by a Registry staff member and entered into the PCOR-QLD Registry, which will be transferred to the binational PCOR-ANZ database.

We will contact you by mail or telephone at 12 and 24 months after diagnosis, when a Registry staff member will confirm your treatment details and ask for your responses to a quality of life questionnaire.

The PCOR-QLD and PCOR-ANZ databases are housed within a secure environment within Monash University. Although the data is identified, your identifying details will never be released outside the Registry.

Participation in PCOR-QLD is voluntary. You can choose to opt-out of PCOR-QLD at any time and your decision to take part or not to take part, or withdraw, will not affect your relationship with your treating hospital or specialist, or incur any out-of-pocket expenses.

If you would like to opt-out of the Registry, please complete the Opt-Out Form (attached to the Participant Information Sheet), call the free-call number 1800 771 410 or send an email to pcorqld@qut.edu.au
